This is a tracking sheet that can be kept in a student portfolio. In my classroom, we used it for tracking the number of Zearn lessons completed each week.
This activity is meant to a fun way for students to practice writing their math facts. Students are instructed to write their facts (I do different facts per week, for example in the blank I put x4 facts or x5 facts, etc.) in white crayon then use a colored marker or crayon (marker worked best for me) over it to reveal their equation. You can also use these to write spelling words.

This is step by step criteria for successfully completing standard algorithm for addition and subtraction of 3 digit numbers. This can be put in the student's hand to use as a reference for their own work. It can also be given to parents/guardians to help their child with work at home.
This page allows students to keep track of their success in multiplication facts. In my class, students would test on each math fact and as they passed them they would color in each number. This was put into Leadership Notebooks but can also be used as a great talking point during parent teacher conferences.
