This is a homework sheet to give to clients working on conversation/pragmatic skills. Have them practice starting/ending conversations with people in their community (at the grocery store, at school, etc.) and then self-evaluate to reinforce and improve their skills. This can be paired with reinforcement; if the client does well in their conversations then they can get a reward.
