This form can be used either as a student-led conference form or as a sheet that can be included with a report card. This form asks students to evaluate themselves on six different areas that are vital to academic success. Students use this rubric to evaluate their success within the classroom. Then, they create goals for themselves based off of their scores.

This 26 page packet includes all the sheets you will need to begin a student portfolio for each child in your class. This packet allows students to collect work samples throughout the year to show parents during student-led conferences. In addition to this, this packet includes Writer's Workshop dividers and activities for the beginning and end of the school.
