This is a chart used for students as a motivator to stay on track throughout the day and make good/positive choices. It also allows the teacher and parent keep track when the student is having a routine of being on their best behavior and their not so good behavior. I have used this to help keep track of documentation when I am setting up a behavior IEP and just to help a student and their parents see what they need to improve on.

We often see Reader's Theater with fairytales and fictional stories. I thought it would be fun to take a nonfiction passage and create a Reader's Theater so that the kids can not only practice their fluency, but also enlighten the rest of the class with useful information. This weekly reading log is geared for primary grades. It allows the student to write down what was read and allows them to give it up to 5 stars. At the bottom of the chart are questions that allow the student to tell which was their favorite and if they read it by themselves or if they had someone read it to them. The last questions gets them to thinking about their next reading log and what book they would like to read.
