This is a game I created to support students as they learn how to create and interpret arrays, which correlates with Common Core Standard.2.OA.C.3.
Students can do this simple dice game independently or with a partner. I have included two different game sheets for you to choose from depending on the needs of your students.
Here are the forms that I use to keep track of student data, grades, attendance, anecdotal notes, and student project ideas. I use all of these forms often and they really help me keep track of everything going on in my classroom. I find these essential to each school year and I hope they help you as well!

I use this anecdotal notes sheet to track how well students have grasped various standards or concepts. This product is editable in case you would like to populate the headings with different criteria and/or add your students' names.
This place value worksheet involves base 10 blocks and a place value/ HTO chart. There are 2 pages- I use one to model with and give the students the other to complete by themselves.
This Anecdotal Notes Sheet is very basic and can be used for any unit/subject/grade. You may choose to populate the categories on the computer or hand write them in yourself.
I created these data sheets to involve students in their own data analysis and goal setting. There is a reading data sheet and a math data sheet. I provided word documents so you can customize them to your own assessment types.
