This bookmark can be given to the students or for teacher use only. Depending on your students, if you choose to give them this bookmark with their level listed be sure to put on cardstock and laminate.

This worksheet would be most useful for upper elementary. It allows students to think about themselves as a reader. It's great for a first week of school activity. I use it after every reading unit when I confer with students to see if they have changed as a reader and if they have new goals. Students can write inside the boxes or use sticky notes to interchange each month, unit and/or quarter. Students can keep it in their reading folder or turn it into the teacher to keep on file.
