When you go hand out all that IEP paperwork to all the required personnel, it helps to have them sign for it so that it is documented. This is an editable form so you can personalize it with your school name and district, and you can change the personnel that is listed if need be. Some districts require that different people have certain paperwork.
This is a simple yearly calendar that can be used to keep track of when those IEP meetings come up throughout the year. I put mine in a page protector and used the little post-it or page markers to place them on there because they can change. I hope it helps you :)
I made this because I couldn't continue searching for what I wanted anymore. I did not want to print out a monthly calendar that had 7 days of the week on it when I was only going to use 5 (or less). So I made my own and here it is! I added only weekdays so no more empty weekend boxes. There are boxes for the dates so the students can write them in, you can print it out, write in the dates, and then copy it. Of course, since it is editable, you can opt to type in the dates, print and copy. Eithe

Students can benefit from evaluating their own behavior. Sometimes, it needs to be more visual for them. I created this to use with my students who know what our classroom rules are and we use the smiley face system in their take home folders so this was easy for them to understand, even if they couldn't read the words. It helps them see what they are doing right, what they are struggling with, and how the teacher might see things differently than they do.

This is similar to another calendar page that I posted that is editable. This one is not editable, but it includes all 12 months of the year. There aren't any specific dates on it so it can be reused each year with no problem. Without the specific dates, you can have the students write them in or the teacher can write them in and then make copies of it.
I hope this can be helpful to you!

Student conferences are important and should be happening with each student periodically throughout the school year. This resource can help you remember how often you've spoken with each student, what it was about, what solutions or outcomes were discussed, and if a follow-up is necessary and/or if it happened. These can be useful for recurring behaviors with students, for academic support for students, or for parent teacher conferences. You choose what and how you use them in your classroom.
I am offering this resource for free for the month of April because it is testing season for us that administer the STAAR Alt. This is a scheduling tool that I have used to keep track of all of the STAAR Alt tests that I have to administer during testing season. STAAR Alt is a test that is individually administered to each student. This tool could be used for other assessments that are administered individually if needed as well.
