When you go hand out all that IEP paperwork to all the required personnel, it helps to have them sign for it so that it is documented. This is an editable form so you can personalize it with your school name and district, and you can change the personnel that is listed if need be. Some districts require that different people have certain paperwork.
I made this because I couldn't continue searching for what I wanted anymore. I did not want to print out a monthly calendar that had 7 days of the week on it when I was only going to use 5 (or less). So I made my own and here it is! I added only weekdays so no more empty weekend boxes. There are boxes for the dates so the students can write them in, you can print it out, write in the dates, and then copy it. Students can benefit from evaluating their own behavior. Sometimes, it needs to be more visual for them. I created this to use with my students who know what our classroom rules are and we use the smiley face system in their take home folders so this was easy for them to understand, even if they couldn't read the words. It helps them see what they are doing right, what they are struggling with, and how the teacher might see things differently than they do.

This is a one page document that can be used for a snapshot of any student who has an IEP or even for a student who has 504 services. These are good for quick reference, to give to general education teachers, or for substitutes.
