This one page incident report can be used for documentation of almost any event that may need attention later. It can go in sub folders or just keep track of any fights or harassment the regular classroom teacher handles in case those events escalate in the future. You can never have too much documentation!

Each year my class does a Newbery medal project where they must choose a Newbery winning book to read and write an essay on. This is the list of Newbery winning books I will send home with students as they look for and choose their book. It includes the award year, author, title, Accelerated Reader reading level, and AR point value. Some titles contain notes from me specific to my class. Based on the ARI/Strategic Teaching strategy of having students rate their knowledge of vocabulary before defining and working with the words, this editable trifold allows you to set students up to monitor their own learning. I often use this as a beginning activity and an after activity in one lesson when introducing new vocabulary. It also works incredibly well as a study tool before the test. Edit for any unit!
