This can be used in your special education or regular education classroom as a resource for student's with behavioral disabilities. If a student exhibits major unsafe behaviors, you should always have a team and a plan. This is a great template to use in a safety plan team meeting. It comes with example responses you can choose to select from to develop your own plan.
This bundle has 15 pages of self monitoring checklists for ALL ages that you can use for a variety of behaviors, special education and general education. It also has a variety of data collection forms you could use to collect data for a behavior plan/FBA or an IEP.
