A parent teacher form that can be used to help with scheduling. Form will provide options for parents to sign up for an email, phone call, or face-to-face conference. Additionally, parents will be able to select the time slots that fit their schedule. Once you are promoted to make a copy of the form, you may edit the times or dates to fit your own schedule.
