This is the current standards I am using in my Physical Education senior classes (Grades 7-12).
Set up so that students set their baseline test at the start of the year and even if they can't hit the excellence they can still improve their mark and in correlation their overall fitness levels.

Simple way to decide warm-ups for the days class. Students roll 2 dice and do the corresponding workout/warm-up. Made a large size for myself to introduce more variety in the students warm-ups each class.
Simple trackers for both the teacher and student to use to keep track of assignments and when to hand in. Wonderful organizational piece for students to remain on top of their assignments.

This is used for self-assessments of the students after a bad day during Physical Education class. Students self evaluate themselves that day to identify what they did right and what they did wrong in that class. Simply and easy to use to help the students self identify any problems they may be having with their attitude, effort, sportsmanship, and/or participation during that class.
