This is a weekly behavior chart I created based upon the needs in my classroom. I use it in specific cases for students needing closer monitoring. This could also be used in conjunction with a "color chart" too, by coloring in the section with the faces. Please note that these documents are intended for individual use only.

This chart is an easy-to-use resource to check your students' assessment level or chart their progress. The most common data points for reading level are shown, along with STAR (Renaissance Learning), F & P/GRL, and Reading Recovery.

This Word doc will allow you to get to know your students at the beginning of the year. It also has a question for the student to ask the teacher something too. I put a sailboat icon on it to go with my nautical-themed classroom, then asked the kids (I have 3rd grade) to color the page, decorate it, draw a picture, etc.
