This file contains a template for a Math book report. I used this as a Math center. Students visited the Library and choose a Math book to read. Then they completed the book report. The pages are to be colored and cut out to form a sandwich. The questions can be seen in the preview. I included the same pages without the instructions so that it can be used for other types of reports.
