I made this sheet as a way to hold my math students accountable for their learning behaviors. It's a report card for them to fill out about their effort and behaviors. The report requires students to rate themselves on 11 math learning behaviors. There's also a spot for students to write a comment about their recent performance. This is a nice activity to accompany teacher report cards at the end of each marking period and/or to be used as a progress report mid-quarter.
