Fifth Fun

This is a handy little sheet I send home at the beginning of every school year. My students are always excited to bring home "homework" for their parents to complete, and these are full of very useful information! This sheet opens up a line of communication between home and school immediately. I can't count how many times I pulled these sheets out of my desk to reference them throughout the school year.

These are a few forms I have used for student-led conferences for my 5th grade class. The following items are included in the document: - A student self-evaluation form - A student reflection sheet - An outline for students to use during their conference These forms mention 5th grade, but can be easily adjusted to fit other grade levels.
