Fifth Fun

I give this form to my students on the first day of school. It's a great way for me to start getting to know them. It helps me get an idea of their family situation and their attitude about school, and it also gives me a chance to learn about some of their favorite things. These sheets are great to reference throughout the school year. They especially come in handy around a student's birthday if you do anything special for birthdays in your classroom!
