I use this sheet with my students on a monthly basis. They set goals for themselves, I post the form in my classroom, and they work to achieve their specific goals over the course of the month. It holds them accountable for what they need to improve upon, and allows them an opportunity to think critically. Since implementing this form, my students have become more motivated to reflect on their learning and figure out where/how they will improve.

Teaching style

I want my students to understand that learning means challenging themselves to work to their maximum potential every single day. I encourage them to take risks and to step out of their comfort zone. I consistently stress that some of the most important learning occurs when we make mistakes. The process of figuring out what went wrong, and fixing a mistake to be correct is a powerful experience!
