Here is the business card that I used to give out to principals when applying for a new teaching position. It has a place for your title, name, picture, email, phone number, online portfolio website URL, and a QR code for them to scan to go directly to your website. This is an individual behavior plan for students with difficulty controlling themselves. It has sections for each subject area/time period of the day. It is great for positive behavior plans because if the student earns 5/8 smiley faces throughout the day, they can earn a reward of their choice.

PANTHER PRIDE behavior monitor can be used for any school's mascot. If your schools mascot are the Eagles, it can be changed to Eagle Pride Behavior Monitor. It has 13 behavior codes that can be found in the key below in the document. They get all of the letters of PANTHER PRIDE at the beginning of the week. If they misbehave after a warning they lose a letter. You write the number under the letter that corresponds to the behavior code. If they lose more than 3 letters in a week, they cannot joi

This is a Password log to keep all of the accounts you use daily in one place for the students. This will be very helpful for parents as well.

The students can move their clips based off of the behavior shown through out the day. They can move up and down. This goes perfectly with my PANTHER PRIDE behavior monitor sheet. :)
