After doing a lesson on -ed and -ing endings this was a sheet used to practice applying the rule. This was for students who receive phonics instruction in a small group center.
BOMT is amazing! We read it in small groups in grade 6 although its a grade 5 text.
This lesson is done to match the standard addressing how characters change in response to events or conflicts with plot. I recreated this chart on chart paper.
The first two columns are to be done while reading part 1 of the the change he has gone through. book. The middle is to make predictions of what will happen once he is hit with the snowball. The last 2 columns are to demonstrate

This is a chart to be filled out during reading so students can organize the many different characters and ships in the book. It can be used as an extension activity or during reading. My students liked using it since they were mixing up characters and it helped them stay organized. They folded it in half and kept it in the book. On the back is an additional drawing space for students to illustrate their favorite scene with dialogue, which can be done at any point.

I usually do a separate spreadsheet for the service grid, but this is helpful so teachers know what student goals are and accommodations they should be providing in the classroom.
Copy and paste for as many students as you have. I use the same template every year.
Students can be given coins or bring in their own for this activity.
They will record the dates on the coins, create a frequency table and then create a histogram using the data.

This blank organizer includes 10 words from Because of Mr Terrupt. The organizer allows space for part of speech, definition, and a sample sentence created by the student to demonstrate understanding.
On the second page of the document is the actual "kid-friendly" definitions of each word to be given to students for a reference afterward. This document is editable.

Balanced Literacy, English Language Arts, Vocabulary
Throughout the book BOMT students were expected to "own" or "master."
Vocabulary: genuine, tolerate, predicament, intervene, shun, collaborate, billow, reassure (I created all my own stuff for vocabulary - see in my products)
This quiz has 10 vocabulary questions and 10 comprehension questions. I used this for a lower tiered novel group in grade 6 but 4th through 6th grade could certainly use it and it can be edited for anyone!
I gave it after reading March.
This is a game with directions at the top to practice reading ou/ow words. They are in sentences/phrases. Students need two colored counters and a dice to play. Used after learning the rule.

During a 4th grade immigration unit, a small group read The Matchbox Diary instead of a lengthier novel. This editable quiz was used to assess student understanding of what they had read/listened to.
This was a quick review for students in my pullout group who previously received phonics instruction and needed additional review. We have a new page to our binder each week that focuses on our skill of the week. There are words to read, sentences to read and room for dictation.

One side of the paper is a dollar words warm up where the kids get to practice the game Mr. Terupt introduces in September.
They can also use this as an ongoing chart to record dollar words as they read.

Wordly Wise Book 4 Lesson 3 Vocab Game
I have, Who has? Cut out cards and pass out randomly to students in small group.
Start person reads their card, next person has to pay attention and read the definition, this goes on until finish card.
Log paper to keep in binder to keep fluid communication between SPED teacher, teacher, paraprofessionals and other service providers. Good way to keep track of students who take breaks and plan for future needs. **Editable/Customizable
While reading Maniac Magee with a small group, I decided to focus on 8 words only while reading. Even though we discussed other unknown words as we read.
Attached is a blank template for student's to write the "kid friendly definition," part of speech, and a sentence using the word.
There is also a definition page with the answers I gave to students who didn't have good definitions.

Short a Game Directions
1.) Draw a card
2.) Read your card, if it has a dog bone take the dog picture
3.) When the timer goes off, whoever has the dog picture wins! or you can play until cards run out and the last person holding the dogs wins
• Copy words onto cardstock then cut out
• Practice reading from the word list at the end of this document if you want to get ready for the game (not part of the game) but can be used to see if they are ready

Students roll dice, then sound out words under that number
After they read those words, they cover up that number with a counter or color it in (depends if you print copies or copy on cardstock)
First player to get all the numbers wins
If they roll a 6 they can skip their turn or roll again
