This is a rubric I use when assessing grade 3 or 4 paragraphs. It looks at topic, details and grammar. I simply highlight their level so I can see it clearer. As I've copied 2 on one page, you could attach this to the students work.
I've complied a sample of what would be said at an Identification, placement and review commitee. Some of the notes include possible points to discuss such as strengths, needs and accomodations. This can easily be altered to be used with a student in any grade.

I use this rubric to assess my grade 4 recount stories. (checking for the main idea, organization, transitions, conventions)I sometimes give the students the rubric and have them assess their own work.
I like sending a newsletter home to parents after the first day of school to introduce myself and how things run in my class. This is a sample of a September newsletter to parents.
I use this daily behaviour sheet to help kids monitor their behaviour. At the start of the day we establish a goal for the day and I fill in the blank. At the end of the day I circle one of the emoticons based on their behaviour. This can be sent home daily for parental signature or at the end of the week.
This is a newsletter I send home to parents on the first week of grade four in preparation for the coming year.
