About the store
I am a passionate special education teacher at heart. I worked for a large district for several years before moving to a private school and starting a learning resource program. The school had no formal paperwork, forms, documentation, or record sheets for anything related to special education. Coming from a huge district where paperwork can be overwhelming, I created forms specifically to document what we needed but kept teachers in mind. The forms are to the point, not overwhelming, and are easy to use. After several years of edits and simplification I am certain that these forms can help schools, classrooms, and teachers save time, and be confident in proper documentation.

Student Input Form: Parent/Teacher Meetings
This form can be used to plan and prepare for parent conferences.
This form would be given to the student prior to a parent teacher conference, IEP or RTI meeting. The students can provide written input behavior and academics strengths and weaknesses.
As the Learning Resource Teacher at my school, I hold annual review parent/teacher meetings for all students on academic support plans or students that were on intervention. I had students complete this form before the meeting. I was able to

These forms are perfect for private & religious schools that do not have district forms and paperwork for exceptional education, accommodations, disabilities, RTI and/or IEPs.
This form documents an ADHD diagnosis and outlines suggested classroom accommodations from the doctor.
