Peace Place - Talking About Problems Independently
This is a great peace place script that allows students to talk about their issues by themselves. Conflict is brought up and talked about in a quick and easy way.
The layout is in a sequence map (flow map from Thinking Maps) with arrows directing students through the script.

This is an extensive kindergarten readiness questionnaire. It has been compressed to only a single page so as not to overwhelm parents. It asks for information about early childhood skill acquisition, preschool history and stress reactions.

Additional biographical information
Prior to teaching I was an art director at a design agency. This background experience allows me to create professional pieces that are printable on common school printers.
