This is a sheet I made based on the most common questions my ESOL parents ask me: "Is my child being polite in class?" "Does she know how to ask for help?" Does my kid understand what they ask them to know?" I fill out this sheet every month and so far the parents feel more and more comfortable knowing all about it.
