This Word document can be edited and changed with different goals. Feel free to make goals with the student and create manageable rewards that the student can get after collecting a certain amount of points. This allows students to be aware of their progress and stride towards accomplishing certain goals. Students can manage and monitor their own success!

This survey is designed for ESL/ ENL students but can be used for any student since all students typically have familial ties to other countries. Students can take the survey independently or the teacher can facilitate more of an interview by orally asking the questions and taking notes on the sheet.
