A tool to help students write about their reading. The format requires students to summarize, synthesize, and evaluate fictional texts. The directions give a detailed explanation of what to include in each paragraph of the essay. The essay consists of 4 paragraphs. A monitoring and intervention plan for struggling students. On the form there is space to write in the areas of need and there is also a checklist of suggestions for support. This form can be used during parent teacher conferences to inform parents of their child's needs and a plan for how to support the child at school and at home.
