I use this form during my reader's workshop to record notes while conferring with individual students. Under the student's name, I record the text they are currently reading, I also circle E (easy), J.R. (just right), or H (hard) referring to this book choice. I then record something positive they are doing as a reader, the teaching point of our meeting, and notes regarding the meeting, future meetings, ideas for future strategy groups, etc.
