If you are looking for a survey for your online students, this is a good one! I usually assign this during the last two weeks of class and give points for completion. In this way, you get the most response from your students. I also set it to anonymous in the program - Angel would let me mark the responses as anonymous but show me who had completed it for the points.
This sheet is my list of things that I expect students to always check before turning in their work. Things like labeling, simplifying, etc. This way, students know what I expect and that they can lose points on assignments for not completing work to these specifications.
Teaching online you may find that you have students who do not reside close to your school's testing center. This form allows students to find a local testing center so that they may complete exams in a proctored setting. It includes forms for the students to complete and forms for the local testing center to complete, as well as space for the instructor's information.
This handout reminds students what they should always do before turning in their work. Always simplify, label, use correct notation & terminology, etc. It is a colorful page that I have my students keep in the front of their notebook so they remember that they will lose points on assignments for not following these rules.
This also works well uploaded for online use where students can access it. It certainly helped me with those "why did you take off points?" questions!
