I use these checklists as quick sight word assessments. This document includes 5 Dolch sight words lists- Pre-primer, primer, 1st grade, 2nd grade, and 3rd grade. I have each of my students go through the list with me about every other week and they check off the words they can read as they go. I put the day of the assessment in the box at the top of the column and total words read at the bottom. We don't always go in order! Sometimes I randomly point to words until we go through them all. These cards are about 2"x1". I use them in my classroom Velcroed onto bowling pins. I just stick velcro dot on the back of the laminated card and choose which 10 words/blends/letters/numbers we're going to practice during that session. As a resource room teacher, I use this log to keep track of my regular communications with the general education teachers. This way I can make sure that I'm regularly checking in on them. It also provides me a record of communications for when general education teachers claim to not know or aren't aren't providing necessary services to my special education students-- I can go back to the log and show them exactly when we discussed it.

Teacher- and student-friendly data collection form. In my resource room, I train my students to fill in this form and collect their own data while participating in interventions. They simply make a tally mark in the check box for correct answers and a tally mark in the x box for incorrect answers. They love being able to see how they're doing!
