This apple themed data packet will help you keep track of your student's progress. This packet includes a title page and pages to document number recognition, rote and one to one counting, upper and lower letter recognition, letter sound recognition, shape and color recognition. The file can be edited to meet your individual needs. Data notebooks are great for parent/teacher conferences.

This is part of the student data notebook. The sheet has all the shapes drawn for the student to look at and a column for you to document what day they could name the shape. Next is a column of different colored smiley faces. Students are to name the color. You can document the day they correctly named the color in the next column. Great for parent conferences.

Next they describe their behavior, why the rule they broke was important and why they broke it. Then they write about what they could have done differently. I give this letter to parents during open house as an introduction to myself and the expectations of our class. It is written for a 5th grade class but it can be edited.

This sheet is filled out by the student when he or she forgets to do their homework. It has a place for the student to explain why the homework is not completed and what they will do to correct the problem. There is also a place for you to list a consequence and a signature area for the student, parent and teacher.

When the class earns a compliment or follows direction well have them click the random number selector. Whatever number it shows is the number they cover up. When they earn bingo they earn a classroom reward of your choice.

Students use this sheet to graph assessment data to show growth. Great for conference and student data notebooks.
