Used as a culminating activity for a data management unit in Grade 4. Students should have learned how to create a survey and also how to draw a bar graph from a set of results.
A simple one-page document that is useful for jotting down notes for a parent-teacher interview. The first table is for putting assessment scores (reading levels, WRAT test scores, etc.) It can be edited to include other types of assessments. There is a section for academic achievements or concerns, as well as a section for behavioural notes. The last section is for jotting notes or goals for moving forward in the school year.
A weekly goal sheet for students to either complete on their own, or with the guidance of their teachers. There are columns for all core classes, as well as Music, Art, French and Phys. Ed.
