A rubric with questions used for with my grade 9 Math students to show their understanding of operations with rational numbers. I use this as an observation/conversation assessment, whereby the students take me through the steps for solving on the board while orally explaining their thought process. Could be edited for other math problems/grades.

5 grammar exercises to be done as a daily exercise at the beginning of each English class. Helps to build basic grammar skills and to practice new concepts. Occasionally I would give a review quiz at the end of the week.
Simple quotations quiz - I had students rewrite the sentences on a lined piece of paper, making sure to put the quotations marks and other punctuation in the appropriate places, as well as paying attention to capitalization and accurate spelling.
Brief handout explaining the assignment of writing "Two Truths and A Lie." I use this assignment as the first creative writing piece in the first week - it allows me to see the students' basic writing skills, as well as getting to know them.
