This is a Parent Teacher Conference Form we use in 3rd Grade to help guide our annual conferences.
The first section at the top is for Student name, and the parents in attendance.
The next section has a place to check the student's beginning of the year reading level- (Below, On, or Above), as well as math level (Below, On, Above).
Then it has a section for 5 subjects and grade on the 1st Progress Report, as well as Current Grade. Then there is a box to comment on the subject/grades.

We use this as a fun, educational way for 3rd graders to understand the new Food Guide Plate Guidelines and how to make healthy choices.
We have students draw a slip of paper to designate their food group (Fruits, Vegetables, Healthy Protein, Whole Grain, and Dairy). Then we give them a paper plate and these directions and they have to make a food item that fits into their food group.
