This is a Parent Teacher Conference Form we use in 3rd Grade to help guide our annual conferences. The first section at the top is for Student name, and the parents in attendance. We use this as a fun, educational way for 3rd graders to understand the new Food Guide Plate Guidelines and how to make healthy choices. We have students draw a slip of paper to designate their food group (Fruits, Vegetables, Healthy Protein, Whole Grain, and Dairy).
