This is a 1 sheet guided reading conference sheet. You are able to note take before during and after guided reading. It is simple to use and can be used to record data of several students quickly.

This is a 6 page file that has the supporting targets that are addressed in each lesson for module 1 unit 3. each target has the CCSS stated.This is basically the "thumb-o-meter" protocol in written form and an area for students to write their evidence/ questions. I usually use this as an exit ticket after each lesson.

This is a 12 page PDF document that consist of Expeditionary Learning Module 1 Unit 2 Learning Targets. They can be used by students to monitor their understanding of the learning targets, kind of like an exit slip. They can be used for individual student portfolios or students can cut them up and paste them on their notebooks before the lesson begins and then reflect on the learning targets when the lesson is finished. Each learning target has been aligned to the CCSS.
