Speech Language Pathologist Data Collection Forms

I originally created these data sheets when my school district began implementing the Charlotte Danielson Framework for Teaching. This model requires you to submit artifacts to prove that you are meeting the standards for each domain of the model. Though I take data on a regular basis, I did not have a well-organized means of collecting data for each student’s individual goals that was easy to keep track of. Creating these individualized data sheets allows me to keep all of my data in one pla

Category Sorting Packet

This packet is designed to be a resource for work on sorting pictures into categories. It can also be used to informally evaluate a student’s category knowledge in order to plan future therapy sessions. Pages are provided for sorting, but I also recommend being creative for how to have the children sort the pictures. For example, rather than placing the cutout pictures onto the papers for sorting, you may want to laminate the category names and cutouts and place magnets or Velcro on the back so

This is an activity for small groups or an entire classroom that is Halloween themed. It focuses on teaching the concepts of MORE and LESS. The teacher indicates how many markers to put next to each Halloween character by spinning the spinner that is included in the packet, or by rolling a die. The students place the corresponding number of markers on their mats, and use their visual to describe which character has more or less markers. This is a great activity for initially teaching these c

Following 3-Part Directions

These direction cards are meant to be put together like a 3-piece puzzle to create a sentence. The pieces should be cut out and glued to the corresponding colored paper so that each puzzle piece is backed with the color of the border. Pieces should then be laminated to make them more durable! You should have a pile of purple directions, blue directions, and green directions set up in that order. The first part of the direction gives an action, the second part gives an object to be used, and

This packet contains 3 types of directions: 3-step directions 2-step directions conditional directions (if/then) Each set contains 18 cards printed on fun Halloween-themed backgrounds. I suggest providing each student with a bag or pumpkin basket to collect cards in. You can have them say "trick-or-treat" before receiving a card. It is also fun to hide the cards around the room and have the students find them. Make sure they complete the directions before putting the card in their basket.
