This document is used when I conference with my students about their i-Ready data.
I find that students put forth more effort when they are aware of their progress and are have ownership of the data.
I keep one of these for each student in my data binder, and every time they complete an i-Ready assessment, we conference over the results.
I use this for the overall scale score, but they may also be used for domains, percentiles, etc.

This template helps me organize sub plans. Aside from the lesson content and schedule, there is a table for dismissal methods, considerations such as IEPs and behaviors, and room for feedback.
My subs have commented that they love the format!
Teachers have also told me it makes putting together sub plans much easier :)
I designed these when I noticed students rushing through their independent work and making errors in the process. Students are to fill these out and attach them to whatever practice they have been assigned.
It helps them take more responsibility for their work, as they have to reflect on what they are about to submit.
It is also a useful (informal) assessment tool for me.
