All teachers and therapists are required to be informed of a student's testing accommodations and program modifications. This sign-off sheet is to be filled out at the beginning of the school year as well as after an annual review. It is important to track how much support a student requires from the paraprofessional that is assigned to work with them. The ultimate goal is to reduce paraprofessional support by fostering increased independence. A chart that I use to organize the important dates of the year for each of my special education students. It is important to know when an annual review or a re-evaluation is due by. Testing Accommodations are hard to keep organized. This form filled out and glued on the front of a folder will make your life easier! Include the proctor's name, the location, and the students taking the test on the form. Inside, put the copies of the test along with the confidential Testing Accommodation Organization Chart.
