This sign-off sheet is to be filled out at the beginning of the school year as well as after an annual review. Testing Accommodations are hard to keep organized. This form filled out and glued on the front of a folder will make your life easier! Include the proctor's name, the location, and the students taking the test on the form. Inside, put the copies of the test along with the confidential Testing Accommodation Organization Chart.
