This letter can be edited and sent home with parents when you first send the Parental Notification and Consent Form to your English Language Learners. This one is geared towards Arizona, but you may edit it to your liking. This letter is two pages, but I have only included part of one page in the preview to protect it from being copied without purchase. When you print it, I would suggest printing double-sided.

This is one of the forms that the state DOES provide online their website, but you are allowed to make changes to it. I thought the one the state provided was not user friendly, and it did not leave room for enough data. So, I created this one. It is editable, so please feel free to include whatever assessments your school focuses on. This goes in the student cumulative file for each student that has tested proficient out of the ELL program. You will need to monitor them for 2 years.

This template is a Notification of Reclassification Form. This will need to be sent home when an English Language Learner scores proficient on the AZELLA. The entire letter is not on the preview. That way, it cannot be duplicated.

Most forms needed for ELL student cumulative files are on the state website.
