This is a slip you would give to your students each time they have missing assignments. The students has to complete the slip by filling in the reason why he/she did not hand in homework. There are also fields for the student and a parent to sign. This slip comes in very helpful when keeping track of students who often don't hand in their homework and to keep parents informed. The idea is to encourage parents to speak to their children about homework and to motivate students.

The Student Responsibility Plan allows students to take ownership of their actions and devise a plan to improve behavior. There are four forms per sheet. I highly encourage printing these on colored paper. I usually have a stack printed and cut in one of my classroom trays. I warn students once of their behavior.
