I use this chart with students whom have trouble following directions. This weakness effects behavior and completion of work in class and at home. I hold on to this (or it is given to the next teacher if the student switches classes) and simply rate the behavior for each class period. Comments can be made from parents and/or teacher at the bottom. Parent initials each night. I check for this, but as long as it is returned on Monday with initials across the week, the student gets to pick from

Have students share their opinions about the books they read. There are two on a page for making more copies available to students. Cut the sheets in half and keep them in the classroom library. They should be readily available for students to fill in. Having them hole punched and hanging on a ring is a convenient way to store the completed ones.
