This template makes me feel secure walking into any IEP meeting, knowing that I accounted for every possible resource, and obtained and analyzed all information appropriate to making a clinical and legal decision. Here is a report I wrote (6/2018) for a kiddo who is in 3rd grade, and had a COMPLEX mental health history, history of destructive and aggressive behaviors in class and at home, and a child who was hospitalized involuntarily (5150) 2x in one school year, for 20+ days at a time

Here is a new report I wrote (9/2018) for a kiddo who is in 7th grade and speaks ONLY Cantonese at home. I describe in detail this student’s dialect variations and what those characteristics look like. This student did not qualify based on his dialect differences. This is a good guide to use for Asian or Chinese

Great information for parents, teachers and students! Visual scale to determine what level a students at, and how to explain carryover into real everyday situations.
