I was searching the internet for a simple way to show tier III?RTI documentation. However, I could not find what I was looking for, so I created my own. I wanted to share it with others in hopes that it helps you too. I suggest you create a folder for each student and place this lesson plan in the folder. You can fill it out weekly and document when you provide each student with planned interventions and the amount of time provided.

Chart to help you plan an intervention to help fill in your struggling readers gaps. This screener is best for 3rd grade and up. I include an example of my small group intervention plans and also provide you with a lesson plan outline to help you create your own intervention plans.
