Give this to a student helper at the beginning of class to complete for an absent student. Make sure the student helper knows to collect any handouts given in class that day for their absent buddy. Have them give the completed form back to you at the end of class. Since I have a Smart Board, I also print a copy of the class notes for the absent student too.
Enjoy!

In the past, I have always left a blank notebook/pad of paper for substitute teacher comments but never really had a consistent form that I used. I often found sub notes were unorganized and sometimes non-existent. After some searching, I found a template online that I really liked.

I decided to re-vamp the form to be a bit more middle school friendly.
