This Google Form includes questions that will allow you to collect basic information about your students as well as get to know them. You can edit it to your liking. I give this during the first day of school or it could be used on the first day of class in a semester-long course. It worked really well with remote learning this year.

I have taught High School Math in Massachusetts since 2013.

I like hands on learning and investigative activities as well as lots of group work and collaboration among students.
My own education history
I earned a BA in Mathematics in 2012 from the University of Massachusetts, Lowell in 2012 and an M.Ed. from the same University in 2013.
Additional biographical information
I have always had a love for math and found my passion for teaching as a tutor in high school. I tutored all through college as well. After earning a bachelor's degree in Math I decided to go to Grad school for teaching. I taught for a a few years at a High School close to Boston, MA and in 2016 started teaching at the High School I graduated from, working with teachers I had and I love being back.
