I regularly check in on my student's reading. I check their home reading logs every morning, I informally ask questions during daily five and, once a month, I do a comprehension conference with them. I use my Comprehension Conference worksheet during such conferences. I ask summarizing, predicting, visualizing, connecting and vocabulary questions. Here's the template I use for the left-side of my Interactive Math Journals. The left-side is used to outline the learning goal, pre and post knowledge, and a self-reflection. Students must follow this format for each and every journal entry. I suggest laminating one copy to keep posted in your class in case a student looses their copy (which should be glued to the inside cover of their journal).
