This test consists of 23 questions (fill in the blank, multiple choice, true and false) and a short answer question about hygiene. It is fairly simple and covers all of the basics.
I use this test for my 4th, 5th, and 6th graders and it is appropriate for each of them.

This document includes 15 statements (and their answers) about tobacco. It is up to students to decide whether they are a fact (true) or a myth (false).
The teacher may choose how they would like students to display their answer (not included). I personally cut out red cards for myth and green cards for facts. You could also do thumbs up and thumbs down, dry erase boards, or writing their answers silently.

24 card "I Have Who Has" game about tobacco. Fun way to review information for students.
Directions: Cut cards apart on the red line (there are 2 per page). Laminate for extended use. Mix up the cards and pass out a card to each student. The student with the first card reads it, and asks the “who has” question. Whoever has the answer to that card reads the “I have” answer, their “who has” question, and sits down.
