Use this tool to break down a task into multiple parts. This is a relied upon tool by those who can see that a simple task for some is a multi-step task/command for others. Extremely handy when pinpointing where a student is finding it challenging to "bridge the gap."

A place to date and journal anything significant that the student in demonstrating during sessions. This can be used to track patterns and trends across sessions so that all instructors working with the student are aware of any changes the student is exhibiting. A tool used to list mastered skills and goals. A row of smaller trial data boxes are given to track how well a student is retaining a specific skill after it has been mastered. A box for the data and the instructors initials can be found in the above column. I love using this tool when monitoring students who show low-high levels of regression.

It can be used to address IEP Goals or other developmental milestones you deem necessary. It allows for anyone working on the program to view instructor and student expectations when running targets in this area. It includes a reference to a method data collection used to teach the skills listed on the STO Sheet (short-term objectives).
