A simple way for kids to keep track of their data throughout the year. It can be adjusted to any title you need!

I created a two page document with questions for my students to answer about themselves. This will be used at the beginning of the year to learn more about them. I usually buy plastic folders at the beginning of the year to use as take home folders, these pages, once decorated, will slip right in as covers, and will make the folders their own. I also keep a copy of the completed pages for myself in my Student Info Binder to refer back to for incentives, goal making, etc.
