I created this modified fitness tracker sheet with the intentions for 5th and 6th grade students to become more independent when recording this fitness scores. If you use the FITNESSGRAM or VA wellness fitness assessments, this can be great for your students you require a bit more simplified language and the use of visual supports to increase independence. Print (double-sided*) these basic school picture cards to assist with student behavior and transitions in and around the school setting. Utilize the ASL signs on the back for students with limited hearing or receptive language skills. Print them out, laminate, and put them on a key ring to take them everywhere you go, provide them for instruction assistants, or specialist teachers to maintain consistency for your students that need it most!
