Are you looking for an individualized behavior plan that allows your student(s) to pick a reward at two different times during the day? This is a simple form that can be used as a daily behavior note, to collect behavior data, and help students have a more positive day. I like to use more positive terms, so I tell my students that they need to have success before they can have rewards. I have included three different versions of the form, so you can use the form that fits your needs the best.

Do you have to write IEPs for students who are self-contained and only see a general education teacher for activities or specials like PE, Art, Music, Computer, Foreign Language, etc.? This for is a useful tool to get their input prior to writing the IEP and having your meeting, so you can be prepared ahead of time. I usually copy it front to back and put it in their mailbox a few weeks prior to the meeting.

This word document is an easy way for you to keep track of your students' IEP goal progress, and is a wonderful tool to share with the IEP team members at your annual meetings! I have left the form editable so that you can copy and paste their IEP goals into the top and choose to type or hand-write the outcomes as you work with each student. I used this form for students with cognitive impairments, but it can easily be adapted for students of any ability.

It contains a daily spot for a book that has been read and parent initials, so it can serve as your reading log and parent communication sheet. We know that students are more likely to enjoy their work if they get to choose what they want to do, which is exactly what this spelling contract does. It has a variety of activities that students can choose to do using their spelling words. I put a copy of this form in a pocket protector in a 3 prong folder and then my students use this as their spelling folder throughout the year.

I earned my Associates Degree in Elementary Education from Broward College, my BA in Sociology from Florida Atlantic University, and received my MS in Educational Leadership from Barry University.
